- Who is supporting you? Are they able to support you well?
- Information about your mental health or physical health.
- Times where you may have caused harm to yourself or others or been at risk of harm from others.
- Have the police or emergency services been involved?
- Are support services or education giving notice to end their work with you?
- What support have you had in the past? What worked well and what did not?
- Misuse of alcohol or drugs
- How well services are working together to support you and include you in their meetings.
We also think about levels of safety
We know that levels of safety can change for better or worse for many reasons.
The professionals involved meet regularly as part of the Dynamic Support Forum to discuss your safety and how well services are working to make sure you are safe.
